The 183 Family Dentistry Membership Plan is not a dental insurance policy. Our membership benefits can only be utilized at 183 Family Dentistry; they cannot be used at any other dental practice. These membership plans cannot be used in conjunction with any other insurance plans, discounts, or promotional offers.
- The membership is for individual use only and cannot be transferred to another person.
- On the date of plan renewal, plans will automatically renew.
- In the event that a patient is diagnosed with periodontal disease, we will formulate a tailored treatment plan at an additional cost.
